Dual aetiologies for hyperinsulinaemic hypoglycaemia in one patient.

چکیده
To the Editor—We read with interest the articles by Ching et al1 and Ma.2 We agree with Ching et al’s recommendation that health care providers should be vigilant to the problem of hypoglycaemia due to inadvertent use of oral hypoglycaemic agents (OHA), and are encouraged to see the increased availability of urine toxicology services for detection of OHA in Hong Kong after the cluster of iatrogenic hypoglycaemia incidents in May 2005.3 However, we also wish to highlight the importance of not overlooking other treatable causes of hypoglycaemia, even in the presence of positive toxicology, by sharing with readers the following case.

Surgical treatment of hyperinsulinaemic hypoglycaemia in infancy and childhood.
Despite a greater awareness of hyperinsulinaemic hypoglycaemia, one in three patients has some degree of mental retardation by the time the diagnosis is made. The diagnosis is established by demonstrating high plasma insulin concentrations during an episode of hypoglycaemia. Twenty one hyperinsulinaemic infants and children were referred for surgical treatment after failing to respond to medica...
متن کاملHNF4A mutation: switch from hyperinsulinaemic hypoglycaemia to maturity-onset diabetes of the young, and incretin response
BACKGROUND Hepatocyte nuclear factor 4α (HNF4A) is a member of the nuclear receptor family of ligand-activated transcription factors. HNF4A mutations cause hyperinsulinaemic hypoglycaemia in early life and maturity-onset diabetes of the young. Regular screening of HNF4A mutation carriers using the oral glucose tolerance test has been recommended to diagnose diabetes mellitus at an early stage. ...
متن کاملPersistent Hyperinsulinaemic Hypoglycaemia and Maturity Onset Diabetes of the Young (MODY) due to Heterozygous HNF4A Mutations
Objective: Mutations in the human HNF4A gene encoding the hepatocyte nuclear factor 4 alpha (HNF-4α) are known to cause maturity-onset diabetes of the young (MODY), which is characterized by autosomal dominant inheritance and impaired glucose-stimulated insulin secretion from pancreatic ß-cells. HNF-4α has a key role in regulating the multiple transcriptional factor networks in the islet. Recen...
متن کاملDiagnosis and management of hyperinsulinaemic hypoglycaemia of infancy.
Hyperinsulinaemic hypoglycaemia is a cause of persistent hypoglycaemia in the neonatal and infancy periods. Prompt recognition and management of patients with hyperinsulinaemic hypoglycaemia are essential, if brain damage and long-term neurological sequelae are to be avoided. Hyperinsulinaemic hypoglycaemia can be transient, prolonged, or persistent (congenital). Advances in the fields of molec...
متن کاملHyperinsulinaemic hypoglycaemia in small for dates babies.
Blood glucose concentrations were measured prospectively in 27 small for dates infants in the first 48 hours after birth: 10 infants became hypoglycaemic. Of these, five had inappropriately raised plasma insulin concentrations. Plasma free fatty acids were lower and carbohydrate intake higher in these five infants, further supporting the diagnosis of hyperinsulinism. The hypoglycaemia recurred ...
